Hospice Home Care in Monticello, AR seeks a dedicated Social Worker to provide emotional support and resource coordination for patients facing terminal illness. You will assess needs, counsel families, educate on advance directives, and collaborate with a multidisciplinary team to ensure dignified, holistic care.

Ideal candidate holds an MSW or CSWE-accredited BSW, has healthcare/hospice exposure, and possess a valid driver's license.

Qualifications

• Master’s in Social Work (MSW) or Bachelor’s in Social Work (BSW) accredited by CSWE
• Minimum one year experience in health care; hospice experience preferred
• Strong verbal and written communication; well-organized
• Current licensure to practice in state of operation
• Must have a valid driver’s license and insured vehicle

Responsibilities

• Assess psychosocial needs of patients and families
• Evaluate home safety; assist with alternative care plans as needed
• Provide counseling, crisis intervention, and emotional support
• Maintain accurate and timely clinical documentation
• Educate and assist with advance directives
• Provide referrals and practical support for patients and families
• Serve as liaison with community resources and agencies
• Collaborate with team to support holistic patient care
• Support funeral, financial, and legal planning with families

Qualifications

• A graduate of a master’s program in social work (MSW) accredited by the Council on Social Work Education OR A graduate of a bachelor’s program in social work accredited by the Council on Social Work Education. Meets personnel qualifications stated in
• 418.114(b)(3).
• Minimum of one (1) years’ experience in health care, hospice experience preferred. Understands hospice philosophy, and issues of death/dying.
• Experience in a hospice care preferred.
• Demonstrates good verbal and written communication, and organization skills.
• Current licensure according to Medicare guidelines to practice in state of operation.
• Must be a licensed driver with an automobile that is insured in accordance with state or organization requirements and is in good working order.
