Job summary

Adoration Health is seeking a compassionate MSW to join our hospice team in Indiana, PA. You will provide emotional support, counseling, and resource coordination to patients facing terminal illness, helping families navigate care decisions with dignity.

Qualifications

• Master's degree in Social Work (MSW) accredited by CSWE or Bachelor's degree in Social Work accredited by CSWE.
• Minimum of one year healthcare experience; hospice experience preferred.
• Current licensure to practice in state of operation (Medicare guidelines).
• Licensed driver with insured vehicle in good working order.
• Proven verbal and written communication and organizational skills.

Responsibilities

• Assess psychosocial needs of patients and families.
• Evaluate home safety; assist with alternative care plans as needed.
• Provide counseling, crisis intervention, and emotional support.
• Maintain accurate and timely clinical documentation.
• Educate and assist with advance directives.
• Provide referrals and practical support for patients and families.
• Serve as liaison with community resources and agencies.
• Collaborate with team members to support holistic patient care.
• Support funeral, financial, and legal planning with families.

This role plays a key part in the delivery of holistic care and ensures that patients' psychosocial needs are met with dignity and respect. Office Location: Indiana, PA

Coverage area: Cambria, Clearfield, Indiana, Jefferson & Blair Counties

Schedule: M-F 8AM to 4:30PM

Qualifications

• A graduate of a master's program in social work (MSW) accredited by the Council on Social Work Education OR A graduate of a bachelor's program in social work accredited by the Council on Social Work Education. Meets personnel qualifications stated in *418.114(b)(3).
• Minimum of one (1) years' experience in health care, hospice experience preferred. Understands hospice philosophy, and issues of death/dying.
• Experience in a hospice care preferred.
• Demonstrates good verbal and written communication, and organization skills.
• Current licensure according to Medicare guidelines to practice in state of operation.
• Must be a licensed driver with an automobile that is insured in accordance with state or organization requirements and is in good working order.
