Job summary

Abode Hospice is seeking a compassionate Social Worker to join our Colorado Springs team. You will assess psychosocial needs, provide counseling and coordinate with families and community resources to ensure holistic end-of-life care.

The role requires an MSW or CSWE-accredited BS in Social Work, with hospice experience preferred. A valid driver’s license and reliable vehicle are required, and you’ll work Mon-Fri, 8:00 AM - 5:00 PM, at our Colorado Springs location.

Qualifications

• Master’s degree in social work (MSW) accredited by CSWE or bachelor’s in social work accredited by CSWE.
• Minimum of one year health care experience; hospice experience preferred.
• Current licensure to practice in the state of operation.
• Must be a licensed driver with a vehicle insured per requirements.

Responsibilities

• Assess psychosocial needs of patients and families.
• Coordinate care and resources with the hospice team and community resources.
• Provide counseling and emotional support to patients and families.
• Maintain accurate, timely clinical documentation.

As a Social Worker You will:

• Assess psychosocial needs of patients and families
• Evaluate home safety; assist with alternative care plans when needed
• Provide counseling, crisis intervention, and emotional support
• Maintain accurate and timely clinical documentation
• Educate and assist with advance directives
• Provide referrals and practical support for patients and families
• Serve as liaison with community resources and agencies
• Collaborate with team members to support holistic patient care
• Support funeral, financial, and legal planning with families

Qualifications

• A graduate of a master’s program in social work (MSW) accredited by the Council on Social Work Education OR A graduate of a bachelor’s program in social work accredited by the Council on Social Work Education. Meets personnel qualifications stated in
• 418.114(b)(3).
• Minimum of one (1) years’ experience in health care, hospice experience preferred. Understands hospice philosophy, and issues of death/dying.
• Experience in a hospice care preferred.
• Demonstrates good verbal and written communication, and organization skills.
• Current licensure according to Medicare guidelines to practice in state of operation.
• Must be a licensed driver with an automobile that is insured in accordance with state or organization requirements and is in good working order.
