Abode Hospice of Colorado is seeking a passionate, dedicated Social Worker to join our team in Colorado Springs, CO. You will provide emotional support, counseling, and resource coordination to patients facing terminal illness, delivering holistic care with dignity.

As a Social Worker you will assess psychosocial needs, coordinate with community resources, educate families on advance directives, and support bereavement and planning.

Qualifications

• Master's degree in social work (MSW) accredited by CSWE or Bachelor's in social work (BSW) accredited by CSWE.
• Minimum of one year experience in health care; hospice experience preferred.
• Hospice care experience preferred.
• Strong verbal and written communication and organization skills.
• Current licensure to practice in the state.
• Licensed driver with insured automobile in good standing.

Responsibilities

• Assess psychosocial needs of patients and families.
• Evaluate home safety and assist with alternative care plans as needed.
• Provide counseling, crisis intervention, and emotional support.
• Maintain accurate and timely clinical documentation.
• Educate and assist with advance directives.

As a Social Worker You will:

Qualifications

• A graduate of a master’s program in social work (MSW) accredited by the Council on Social Work Education OR A graduate of a bachelor’s program in social work accredited by the Council on Social Work Education. Meets personnel qualifications stated in §418.114(b)(3).

• Minimum of one (1) years’ experience in health care, hospice experience preferred. Understands hospice philosophy, and issues of death/dying.

• Experience in a hospice care preferred.

• Demonstrates good verbal and written communication, and organization skills.

• Current licensure according to Medicare guidelines to practice in state of operation.

• Must be a licensed driver with an automobile that is insured in accordance with state or organization requirements and is in good working order.
