Job summary

Abode Hospice of Colorado is seeking a dedicated Hospice Admissions RN for Colorado Springs, CO. You will conduct in-home and facility visits, assess patient needs, and coordinate start-of-service activities with the care team.

The role emphasizes patient-centered care, independent work with team support, and opportunities for professional growth in a supportive environment.

Qualifications

• Graduate of an accredited nursing program with current RN license in the state of operation.
• One year hospice nursing experience required; 3+ years in related settings may be considered.
• Must have a reliable automobile and a valid driver's license.

Responsibilities

• Visit patients in homes, hospitals, nursing facilities, etc. to assess hospice needs.
• Explain hospice services, goals, and care plans with patients and families; obtain consents and documents.
• Obtain medical history, current medications, and symptoms; perform comprehensive assessments for eligibility.
• Coordinate start of services (nursing, aides, equipment, medications).
• Collaborate with interdisciplinary team; maintain accurate documentation.

As a Hospice Admissions RN You Will:

• Visit patients (in homes, hospitals, nursing facilities, etc.) who are referred for hospice care
• Explain hospice services, discuss goals and care with the patient and families, answer any questions, and obtain necessary consents and legal documents
• Obtain medical history, current medications, and symptoms
• Perform comprehensive clinical assessments to determine eligibility for hospice based on Medicare/insurance guidelines
• Coordinate the start of services (nursing, aides, equipment, medications)
• Collaborate with interdisciplinary team
• Maintain accurate and timely documentation

Qualifications

• Graduate of an accredited nursing school with current licensure to practice in the state of operation
• One year of hospice nursing experience required, 3+ years preferred or similar setting such as Long-Term Care, Intensive care, or Emergency room will be considered in place of hospice nursing experience
• Must be a licensed driver with an automobile that is insured in accordance with state/or organization requirements and is in good working order
