Job summary

Angels Care Home Health is seeking a Social Worker (MSW) to provide coordinated home-based care for clients across all ages. The MSW conducts psychosocial assessments, counseling, and makes referrals to meet patient and family needs.

The role requires strong interpersonal skills and ongoing professional development. Qualifications include a Master’s in Social Work, state certification, current driver’s license, and at least one year in a health care MSW setting.

Qualifications

• Master’s degree in social work.
• State Certification.
• Current State Driver’s License.
• One year of MSW experience in a health care agency.
• Working knowledge of community resources.
• Proof of current CPR and Hepatitis profile.

Responsibilities

• Assess home health patients to identify psychosocial, financial, environmental, and community needs as evidenced by POC, documentation, clinical records, case conferences, and referrals.
• Use effective interpersonal relations and communication skills; facilitate their use by other team members to achieve desirable outcomes.
• Meet mandatory continuing education requirements.
• Demonstrate commitment, professional growth and competency by maintaining knowledge of eligibility standards.
• Promote Agency philosophy and administrative policies to ensure quality of care.
