Adventist HealthCare Home Health is seeking a Social Worker MSW for Southern Maryland and Silver Spring territories. Eligible candidates may receive up to a 2,500 sign-on bonus.

The role emphasizes therapeutic care, psychosocial assessments, and collaboration with an interdisciplinary team to support patient outcomes. The ideal candidate holds a Master’s in Social Work with LGSW licensure and 1–3 years of social work experience, preferably in inpatient settings.

Qualifications

• Master’s Degree in Social Work or equivalent Master’s Degree in a recognized health field.
• Licensed Graduate Social Work Licensure Required.
• One to Three Years Social Work Experience Preferred, Experience in an Inpatient Setting Preferred.
• Knowledge of Psychological Disorders & Abnormal Psychology Related to Adolescents and Adults Required.

Responsabilités

• Provide quality therapeutic care appropriate to client age, diagnoses, and mental status.
• Conduct psychosocial assessments and provide individual, group, and family therapies.
• Participate in interdisciplinary treatment planning to promote outcomes.
• Assist clients in developing self-respect, dignity, and self-actualization.
• Document services and share reports with community referral sources.
• Coordinate with interdisciplinary team and resources for discharge planning.
• Assess and intervene in suspected/ disclosed neglect or abuse cases.
• Promote positive patient-family-community relations.
