Zusammenfassung

21st Century Home Health Services (21HHS) seeks a Medical Social Worker (MSW) to provide in-home social work services to predominantly geriatric patients in Santa Rosa and surrounding areas. Most patient visits occur in patients' homes, with about 15–20 visits per week and a primary location assigned.

The role requires a Master's in Social Work, CSWE accreditation, and a valid driver's license. LCSW is preferred; BLS is required, and Home Health experience is valued.

Qualifikationen

• Master's Degree from a CSWE-accredited program is required.
• Basic Life Support (BLS) certification is required.
• Driver license is required.
• LCSW license is highly preferred; supervision is provided for license-eligible Social Workers.
• Automobile insurance is required.
• One year of MSW experience in health care is strongly preferred.
• Home Health experience is preferred.

Aufgaben

• Under the general direction of the Case Manager, assess social and emotional factors related to the patient's illness and medical needs.
• Develop and implement a plan for home social work services including education of patients, families, and caregivers.
• Coordinate with other home health staff and community resources to integrate care efficiently.
