Responsibilities

• Coordinate the discharge planning process for all patients.
• Evaluate psychosocial aspects of illness to optimize treatment and care participation.
• Develop and maintain knowledge of local/state/federal services for patient resources.
• Support HIM and Patient Accounts with accurate data entry across information systems.
• Collaborate with Care Coordinator Physician Services/Interdisciplinary team to review plan of care.
• Monitor progress toward discharge goals and revise plan as patient needs change.
• Ensure post-discharge care plan is transferred to next level of care and documented.
