Job summary

UnityPoint Health is seeking a licensed Social Worker to support patients and families in hospital and ambulatory settings. You will provide psychosocial assessments, crisis intervention, and resources coordination to address barriers to care.

You will educate on Advance Directives, assist with discharge planning, and collaborate with an interdisciplinary team to ensure patient safety and continuity of care. Strong communication and teamwork are essential.

Qualifications

• Master’s degree in Social Work from an accredited school of social work.
• Current LMSW licensure in good standing where services are provided.
• Completion of mandatory training required by state and organization.

Responsibilities

• Performs psychosocial assessment to identify priority needs and barriers to care.
• Provides crisis intervention and supportive counseling to patients and families.
• Refers to community resources and assists with care transitions and discharge planning.
• Educates patients and families on Advance Directives and facilitates planning conversations.
• Documents assessments and referrals in the electronic health record per standards.
• Collaborates with interdisciplinary teams to support patient safety and care continuity.
