Qualificações

• Master’s degree in Social Work is required.
• Three (3) years of professional social work experience is required.
• License as a Social Worker in Texas (LMSW, LSW) is required.
• Excellent communication skills; bilingual (Spanish/English) preferred.

Responsabilidades

• Performs psychosocial assessments, health education, supportive counseling and referrals.
• Collaborates with multidisciplinary team to provide comprehensive care; documents all patient/provider communication and interventions.
• Identifies obstacles to optimal patient care; risk reduction; and health promotion.
• Coordinates ongoing counseling and support groups; participates in program evaluation activities.
