Job summary

UT Health San Antonio in San Antonio, TX, is seeking a licensed social worker to provide psychosocial assessments, counseling, and case management within a multidisciplinary team. This on-site role requires a Master’s in Social Work, 3 years of experience, and LMSW/LSW licensure to support diverse patient populations.

The position includes coordinating counseling services, assisting with resource referrals, and participating in program evaluation.

Qualifications

• Master's degree in Social Work is required.
• Three years of professional social work experience is required.
• License as a Social Worker in Texas (LMSW, LSW) is required.

Responsibilities

• Performs psychosocial assessments, health education, supportive counseling and referrals; assists patients in finding resources.
• Collaborates with multidisciplinary team to provide comprehensive care; documents all patient/provider communication and interventions.
• Identifies obstacles to optimal patient care; loss or grief work; risk reduction; and health promotion.
• Provides clinical case management services including referrals for housing, meal assistance, medical transportation, and other needs as identified.
• Formulates candid and supportive communication.
• Coordinates ongoing counseling and support groups.
• Facilitates effective problem solving and conflict resolution.
• Participates in program evaluation activities including data collection for grants, grant reporting, patient satisfaction surveys.
