Job summary

Ascension Living in Flint, Michigan, seeks a full-time Social Service professional for its PACE Michigan program. The role focuses on coordinating care, assessing participant needs, and consulting with care teams to ensure adherence to treatment plans.

Master's in Social Work is required with 3 years of experience preferred; licensure as MSW is state-dependent and preferred. This on-site position offers day shift hours and comprehensive benefits through Ascension.

Qualifications

• Master's degree in Social Work is required.
• MSW licensure preferred or state-required for practice.

Responsibilities

• Interviews participants, assesses priorities and documents case activity.
• Consults with team managers, statutory and voluntary agencies and participants' caregivers to ensure compliance with the treatment plan.
• Assists participants/caregivers with obtaining community assistance by referral to proper resources.
• Develops and maintains relationships with community health, welfare and social agencies; seeks creative means to assist participants.
• Supports discharge planning, social services, home care and other post-discharge needs.
