Qualifications

• Master of Social Work from an accredited School of Social Work.
• Licensed by the State of Ohio Social Work Board (LSW, LISW, or LISW-S).
• Minimum 1–2 years of equivalent experience; 3–5 years preferred.
• For UC Health Psychiatric Services, Nonviolent Crisis Intervention with Advanced Physical Skills certification within 90 days of hire.

Responsibilities

• Respond to referrals and complete psychosocial assessments.
• Develop and implement a plan of care addressing patients' needs to promote continuity of care.
• Provide education, consultation, and liaison to the healthcare team on psychosocial aspects of care.
• Offer individual/family counseling and crisis management related to psychosocial barriers.
• Facilitate discharge planning and referrals to community resources.
• Maintain timely, clear, and concise documentation; write reports as needed.
