Qualifications

• Master’s degree in social work required.
• 2 years acute care experience preferred.
• Knowledge of community resources and healthcare system.
• BLS certification through the American Heart Association for Healthcare Providers.
• Highly organized, empathetic, and capable of multitasking in a high-stressed environment.
• Strong time management, outcomes analysis, and communication skills.
• Excellent verbal and written communication and organizational abilities.
• Ability to work independently within scope of practice.

Responsibilities

• Provide psychosocial assessments, therapeutic treatments, counseling and discharge planning.
• Care coordination for patients and families, with multidisciplinary teams.
• Review plans with patient, family, and care team and document accordingly.
• Coordinate evaluations among disciplines and schedule family meetings as needed.
• Provide psychosocial interventions and document accordingly.
• Identify community resources and facilitate safe discharges.
• Support patients and families coping with illness, including emotional aspects.
• Negotiate indigent issues and coordinate care transitions.
