Job summary

Jupiter Medical Center in Palm Beach County seeks a Master’s level Social Worker to join the Case Management team, providing psychosocial assessments and discharge planning across DME, SNF, LTAC, and home health agencies.

Under the Director’s supervision, the MSW will coordinate evaluations, craft comprehensive discharge plans, review plans with patients and families, and ensure regulatory compliance during transitions.

Qualifications

• 2 years acute care experience, SNF, or home health experience.
• Knowledge of community resources and the healthcare system.
• BLS Certification through American Heart Association.
• Highly organized, empathetic, and capable of multitasking in a high-stress environment.
• Strong prioritization, conflict management, and time management.
• Excellent verbal and written communication skills.
• Proficient assessment skills for psychosocial needs and transition planning.
• Ability to coordinate discharge planning with multiple care teams.

Responsibilities

• Provide accurate information to DME, SNF, LTAC, HHC agencies and other providers along the patient’s continuum of care.
• Coordinate discharge evaluations and craft a comprehensive discharge plan.
• Review the plan with patient, family, and care team to ensure understanding and agreement.
• Document in the medical record and coordinate family meetings when requested.
• Offer psychosocial interventions and referrals after discharge as needed.
• Understand social and functional factors affecting discharge and outcomes.
