Job summary

Amedisys is hiring a Hospice Social Worker at Masters level to support patients and families in home-based hospice care across Alamance, Orange, Chatham, Rockingham and Caswell counties. This part-time role offers 30 hours per week with competitive compensation ($27-$32 hourly) plus mileage and benefits.

You will collaborate with an interdisciplinary team to address social, emotional and financial needs, coordinate care plans, and ensure timely communication of changes in patient status, in line

Qualifications

• Masters or Doctorate in Social Work from CSWE-accredited school.
• One year of social work experience in a health care setting.
• Current CPR certification.

Responsibilities

• Assists patients and families with health-related financial, social, and emotional concerns and coping skills.
• Provides social work services per the plan of care.
• Communicates changes in clinical status to the health care team and care center staff promptly.
• Educates patients and family members and other staff on social work interventions and treatment plans.
• Performs care planning, transfer, and discharge functions.
• Identifies risks in the patient's environment and implements interventions; coordinates with physicians and team.

Attractive pay

Responsibilities

• Assists the patient and family with health related financial, social and emotional concerns and assists with effective coping skills.
• Provides social work services in accordance with the plan of care.
• Ensures that changes in the patient's clinical status are communicated to other healthcare team members and the care center office staff in a timely manner.
• Educates the patient and family members and other team members on medical social work interventions and treatment plan.
• Performs care planning, transfer and discharge functions.
• Identifies risks or threats to the patient's health in the patient's environment and promptly implements appropriate interventions to reduce risks. Communicates identification of risks/ threats to the patient to the physician and other members of the healthcare team to coordinate care effectively.
• Other duties as assigned.

Qualifications

• Holds a masters or doctorate degree from a school of social work, accredited by the Council on Social Work Education
• Has one year of social work experience in a health care setting.
• Meets state and federal requirements for this position.
• Current CPR certification.
