VitalCaring Group is seeking a Master’s Social Worker (MSW) for field-based hospice care. You will provide essential emotional, psychosocial, and practical support to patients and families navigating end-of-life care.

This role emphasizes compassionate counseling, crisis support, and collaboration with nurses, physicians, chaplains, and social work teammates to ensure dignified, patient-centered care across the hospice journey.

Qualifications

• MSW degree required; hospice experience preferred.
• 1+ year MSW hospice experience or equivalent.
• Valid driver’s license, reliable transportation for home visits.

Responsibilities

• Assess psychosocial needs of patients and families related to terminal illness and end-of-life care.
• Provide counseling and emotional support to patients and caregivers throughout the hospice journey.
• Identify psychosocial risks and offer crisis interventions as needed.
• Assist with advance directives, community resources, and practical planning needs.
• Collaborate with interdisciplinary hospice team to develop plans of care and support.
