Enhabit Home Health & Hospice is seeking a qualified Social Worker to administer social services to patients requiring intermittent professional services, aligned with physician orders and the care plan.

Responsibilities include teaching patients and families, coordinating care, and ensuring compliance with state licensure; a Master's in Social Work and active licensure are required, with at least one year of health-care experience. Ohio drivers license preferred for field visits.

Responsabilités
