Job summary

Enhabit Home Health & Hospice is seeking a licensed MSW to join our Texas team. You will administer social services to patients needing intermittent professional services and collaborate with physicians, families, and the health care team to implement the plan of care.

We require a master's degree in social work, a current state license, at least one year of healthcare social work, and basic technology skills including a mobile device. Medicare home health or hospice experience is preferred.

Qualifications

• Must have a masters degree from a school of social work accredited by the council on social work education.
• Must be certified or licensed in the state where they currently practice.
• Must have at least one year of social work experience in a health care setting.
• Must have basic demonstrated technology skills, including operation of a mobile device.

Responsibilities

• Administer social services to patients requiring intermittent professional services.
• Teach the patient, family, and other members of the health care team; perform services per physician orders and plan of care.
