Elara Caring is seeking a Medical Social Worker (MSW) to join our team. You will assess psychosocial, financial, and environmental needs, develop and reevaluate the Plan of Care, and collaborate with physicians and the care team to ensure quality home-based services.

The role requires a Master’s or Doctoral degree in Social Work, state licensure, and at least 1 year of healthcare social work experience, with travel up to 50%. Strong communication and a valid driver’s license are essential.

Competenze

• Master’s or Doctoral degree in Social Work from CSWE-accredited program.
• 1+ year of social work experience in healthcare setting.
• Current State License as a Social Worker.
• Excellent verbal and written communication skills.
• Social work skills as defined by general social work standards.
• 50% travel required.
• Valid driver’s license, insurance, and reliable transportation.

Mansioni

• Execute psychosocial assessments and document findings.
• Conduct initial and ongoing social work evaluations.
• Communicate significant findings to supervisors, physicians and care staff.
• Contribute to Plan of Care and discharge planning.
• Incorporate patient and family into meeting social work needs.
