Job summary

Elara Caring is seeking a Medical Social Worker (MSW) to join our home health team. You will assess patients, develop and adjust care plans, and coordinate with physicians and the care team to ensure safe, effective discharge planning. This role requires 50% travel and a CSWE-accredited MSW degree with active state licensure.

Qualifications

• Master’s or Doctoral degree in Social Work from a CSWE-accredited program.
• 1+ year of social work experience in healthcare.
• Current State License as a Social Worker.
• Excellent verbal and written communication skills.
• Social work skills as defined by standard practice.
• 50% travel required.
• Valid driver’s license and reliable transportation to perform job tasks.
• Ability to frequently lift, push, pull, and transfer patients up to 50 pounds.

Responsibilities

• Ensure activities align with Elara Caring’s board, executive team, and Home Health leadership.
• Assess patients for psychosocial, financial, and environmental needs and document findings.
• Conduct initial social work evaluation and reevaluate needs at subsequent visits.
• Communicate significant findings and changes to supervisors, physicians, and care team.
• Report unsafe conditions and visit outcomes to the appropriate supervisor by day’s end.
• Implement care plans for patient safety using community resources.
• Contribute to Plan of Care and discharge planning for quality and continuity of care.
• Verify Plan of Care before visits and follow physician orders and guidelines.
• Initiate and revise the Plan of Care in response to issues.
• Write physician orders for additional visits and changes to the plan of care.
• Incorporate patient care goals from the plan of care in documentation.
• Perform skilled services per standards and physician authorization.
• Counsel and involve patient and family in following the plan of care.
