Job summary

myPlace Health, a PACE organization, is seeking a dedicated MSW to join our onsite Los Angeles team. You will plan, organize, and deliver psychosocial services, collaborating with the interdisciplinary team to coordinate care for participants and families in the community.

You will manage a caseload, conduct assessments, develop care plans, and assist with Medi-Cal processes while supporting transitions and discharge planning. Bilingual abilities are a plus as we serve diverse populations.

Qualifications

• MSW degree from an accredited university.
• CPR and First Aid certification required or must be obtained within 30 days of hire.
• 2 years of experience conducting psychosocial assessments, care planning, and case management.
• 2 years of experience working with frail or elderly populations.
• Current California Driver's License with a clean driving record and personal automobile insurance (required by State law).
• Proficiency in EMR/EHR documentation preferred.

Responsibilities

• Attend daily IDT meetings to ensure participants' voices are heard and psychosocial assessments and care plans are developed.
• Present assessments at care plan conferences and collaborate with the IDT to create person-centered plans.
• Guide participants through smooth care transitions between hospitals, homes, and long-/short-term facilities.
• Connect participants, families and caregivers with community resources and provide ongoing support.
• Assist with Medi-Cal applications and coordinate communications with government agencies.
• Document care plans and interactions accurately in the EMR and support discharge planning.
• Lead group counseling sessions and activities to promote emotional well-being and social engagement.
