Qualifications

• Master’s degree in Social Work required.
• Minimum 3 years of hospital experience preferred.
• Knowledge of case management.
• Experience with skilled nursing facility, home health, hospice, child/adult/elder abuse and neglect, and rehabilitation preferred.

Responsibilities

• Coordinate care of an assigned caseload with physicians, nursing staff, and other health professionals.
• Facilitate efficient utilization of clinical resources, optimal clinical and continuing care, as well as financial outcomes.
• Provide a collaborative process of assessment, plan, facilitate, care coordination, evaluate, and advocacy for options and services to meet individual and family needs.
• May have additional duties assigned that are within scope of the role.
