Well Care Health is seeking a compassionate Medical Social Worker to provide in-home social work services. Responsibilities include patient and family education, care coordination, and program development within a clinical framework.

Qualifications

• Master of Social Work from an accredited school of social work.
• Licensed or eligible to become a Clinical Social Worker in the state.
• Minimum one year of healthcare social work experience.
• Ability to communicate effectively in English.

Responsibilities

• Assess social and emotional factors for patients across ages.
• Plan, implement and evaluate social work interventions within a care plan.
• Communicate with physicians and the care team per agency policies.
• Contribute to program effectiveness and service quality.
• Organize workload and maintain scheduling for patient care.
• Demonstrate agency values and maintain customer satisfaction.
• Collaborate in a multidisciplinary home health team.
