Job summary

Well Care Health in Myrtle Beach, SC is seeking a Medical Social Worker to join our home health team. You will assess social and emotional needs, develop care plans, and provide education to patients and families in their homes.

The ideal candidate holds an MSW, is certified or eligible as a Clinical Social Worker, and has at least one year of healthcare social work experience. This role emphasizes collaboration within an interdisciplinary team and commitment to patient-centered care.

Qualifications

• MSW from an accredited school of social work.
• Certified (licensed) or the ability to be certified as a Clinical Social Worker by the State Certification Board for Social Work.
• One year of experience as a Clinical Social Worker in a healthcare setting. Prefer home health experience.

Responsibilities

• Assesses, interprets, plans, implements and evaluates the social and emotional factors related to the patient’s illness.
• Provides goal oriented therapy directed toward the management of illness, strengthening the family’s support and resolving conflicts as part of a multidisciplinary team.
• Appropriately communicates information in accordance with agency policies and procedures and discipline specific guidelines.
• Maintains and promotes customer satisfaction.
• Maintains written caseload and informs clinical manager of patient needs and status.
