Job summary

Qualifications

Responsibilities

• Perform social work assessments and develop treatment plans integrating medical, social, and resource issues.
• Collaborate with healthcare team to implement patient-centered plans of care for individuals and families.
• Involve crisis intervention, high-risk screening, brief counseling, and referrals for financial or other resources as needed.
• Arrange and facilitate family/patient meetings with the health care team and assist in post-acute placement on complex discharges.
