Qualifications

• Master's degree in social work or related field.
• Washington Clinical Independent Social Worker Associate License upon hire.
• 3 years of social services experience within a health care setting, including 2 years in acute care or care coordination.

Responsibilities

• Coordinate care for complex designated patients from pre-admission to immediate post-discharge.
• Facilitate coordination and communication between the health care team, patient and family.
• Provide skilled psychosocial services to patients and families and support clinical staff in psychosocial needs.
• Act as a resource to evaluate patient needs for psychosocial and emotional support during crises and transitions.
