Qualifications

• Must hold a Mastera's degree in social work or related field.
• Upon hire: Washington Clinical Independent Social Worker Associate License.
• 3 years of social services experience with a health care related organization; experience should include a minimum of 2 years in acute care/care coordination.
• ACM or CCM certification preferred upon hire.

Responsibilities

• Coordinate care for complex designated patients from pre-admission to post-discharge.
• Act as liaison between health care team, patient, and family to minimize fragmentation.
• Provide psychosocial services and support to patients and families for crises and transitions.
• Serve as resource to clinical staff in evaluating psychosocial needs.
