Job summary

Qualifications

• MSW from CSWE-accredited program.
• 2+ years Post-MSW experience required.
• Preferred: 5+ years Post-MSW healthcare experience.

Responsibilities

• Conduct psychosocial assessments and develop care plans.
• Provide counseling and crisis intervention and connect clients to resources.
• Advocate for patients and coordinate with multidisciplinary teams for holistic care.
• Address social determinants of health and assist with transitions of care.
• Educate patients and families on care options and community resources.
