Job summary

Seven Counties is seeking a part-time MPW professional to conduct in-home assessments and document health, social, and environmental factors for eligibility determinations in Kentucky.

The role requires a Master’s or Bachelor's degree in Social Work, Psychology, Sociology, or Human Services with appropriate licensure, one year DD/ID experience (or six months with a Master’s), and a valid driver’s license for frequent travel to client homes.

Qualifications

• One year experience with the DD/ID population is required (with a Bachelor’s degree) or six months with a Master’s degree.
• Good interpersonal skills and written and oral communication skills.
• Working knowledge of MPW and eligibility criteria.
• Valid driver’s license and reliable transportation are required.

Responsibilities

• Conducts MPW assessment, including a face-to-face visit with the individual in the home within the 14 day deadline or state-established deadline.
• Assesses and documents physical health, mental health, social supports, and environment using state-designated tools.
• Observes and interviews individuals and representatives to identify needs and services.
• Explains paperwork to the individual or guardian and completes required forms at assessment.
• Provides MPW recipients and/or family with information about MPW and case management providers.
• Submits MPW evaluation to the Quality Improvement Organization for Level-of-Care decision and shares results with providers upon approval.
• Assists the individual with appeal procedures if Level-of-Care is denied.
