The role requires documenting responses with a detailed 15-page assessment tool, obtaining consents, sharing securely, maintaining EMR notes, coordinating Level-of-Care determinations, and communicating with families and case managers to keep evaluations on track.

Qualifications

• Requires RN or SW licensure and a Bachelor's degree as per Medicaid regulation.
• Experience preferred but not required.
• Reliable transportation for frequent travel.

Responsibilities

• Conduct Michelle P. Waiver assessments and reassessments, including a face-to-face home visit within the deadline.
• Complete initial assessments for new MPW participants and advise on service delivery options.
• Document responses using a 15-page state-designated assessment tool covering health, ADLs, neuro/emotional status, social supports, and environment.
• Obtain consents and complete state-assigned forms; share documents securely with individuals and medical records staff.
• Enter information notes and assigned documents in the EMR within deadlines.
• Submit MPW evaluations to the Quality Improvement Organization for Level-of-Care decisions.
• Communicate assessment status with individuals, families, and case managers throughout the process.
