Job summary

Seven Counties Services seeks a Part-time MPW professional to perform home-based assessments and coordinate care for individuals with DD/ID. The role requires master’s or bachelor’s degrees in social services with licensure as required by Kentucky boards, plus relevant experience and reliable transportation.

Qualifications

• Master’s degree or Bachelor’s degree in related field with licensure as required by Kentucky licensing board.
• Experience with DD/ID population preferred; one year minimum with Bachelor’s or six months with Master’s.
• Good interpersonal and written/oral communication skills.
• Reliable transportation and valid driver's license are required.

Responsibilities

• Conduct MPW assessments including home visits within state deadlines.
• Assess and document physical health, mental health, social supports and environment.
• Interview individual, family or guardians to identify unmet needs.
• Explain paperwork and obtain required forms during assessment visits.
• Provide information about MPW and case management providers to recipients/families.
• Arrange MPW evaluation submission to Quality Improvement Organization and coordinate Level-of-Care decisions.
• Assist with appeal procedures if Level-of-Care is denied.

Job Description:ESSENTIAL JOB FUNCTIONS1. Conducts MPW assessment, including a face-to-face visit with the individual in the home within the 14 day deadline, or other deadline, as established by state criteria.2. Assesses and documents the individual’s physical health, mental health, social supports, and environment, using assessment tool designated by the state.3. Observes and interviews the individual and his or her representatives or family members to identify the needs and services that the recipient or recipient’s family cannot manage or arrange on the individual’s behalf.4. Explains paperwork to the individual, and/or guardian, and has them complete the Freedom of Choice form, along with other state and agency required paperwork, at the face-to-face assessment visit.5. Provides potential MPW recipients and/or family members with information about the MPW and available case management providers.6. Arranges for submission of the MPW evaluation to the Quality Improvement Organization, which determines the Level-of-Care approval or denial, and the assessment and/or reassessment to the respective MPW provider upon approval of the Level of Care.7. Assists the individual with the appeal procedures in the event of a Level-of-Care denial and upon the request of the individual or his or her family.The intent of this job description is to provide a representative summary of the major duties and responsibilities performed by incumbents of this job. Incumbents may be requested to perform job-related tasks other than those specifically presented in this description.EDUCATION• Master’s degree in Social Work, Psychology, Sociology, or Human Services with a minimum associate licensure from the proper KY Licensing Board, independent license preferred (if not required by regulation), or• Bachelor’s degree in Social Work, Psychology, Sociology, or Human Services from an accredited university, or a Registered Nurse with unencumbered RN license from the KY Board of Nursing.EXPERIENCE• One year experience with the DD/ID population is required, with a Bachelor’s degree, or six months’ experience with the population with a Master’s degree.• Good interpersonal skills and written and oral communication skills.• Working knowledge of the MPW and eligibility criteria.• Reliable transportation for frequent travel.• Valid driver’s license is required.PHYSICAL DEMANDS• Position has no unusual physical demands.• Position requires travel to offsite locations and may require walking or standing, stooping or bending.• Position requires extensive work in client’s homes, occasionally entailing modestly unpleasant situations.Within the bounds of their respective job descriptions, all staff are expected to exercise principle-centered leadership, focused on customer service responsiveness, with a continuous quality improvement orientation.
