Job summary

A Place At Home is seeking a compassionate Per Diem Registered Nurse for the Morris County area to provide in-home, patient-centered care to clients in their own homes. You will perform assessments, develop care plans, and support families and caregivers.

The role emphasizes caregiver supervision, coaching, and collaboration with clients, families, and the agency, with flexible scheduling and a commitment to high-quality care.

Qualifications

• Active RN license in NJ.
• Bachelor’s degree in Nursing preferred.
• Minimum 2 years of clinical experience, ideally in home care.
• Reliable transportation and auto insurance.
• Clean background and criminal record.
• CPR/BLS certification.

Responsibilities

• Conduct initial and ongoing assessments to identify client needs and develop effective care plans.
• Collaborate with clients, families, and caregivers to ensure seamless transitions and optimal outcomes.
• Conduct caregiver supervisory visits in compliance with state regulations and agency policies.
• Assess caregiver competency and provide coaching, education, and support as needed.
