Job summary

First Choice Home Health & Hospice in Utah is seeking compassionate CNAs to join our care team. The role offers part-time hours, primarily mornings, with weekly case conferences to coordinate patient care.

You will assist patients with daily living activities, grooming, and ensuring comfort and safety while collaborating with a dedicated care team; travel with reimbursement, and a valid CNA license is required.

Qualifications

• Must be available daily Monday-Friday, beginning at 8 am.
• Active CNA license required.
• Over the age of 18.
• Valid Driver's license and reliable transportation.

Responsibilities

• Assists patients with daily living activities
• Spends time assisting patients with grooming to help them feel great
• Works as part of the care team to help patients reach their goals and to be comforted
• Maintains patient comfort and safety
• Attends case conference weekly to coordinate patient care with case manager and team
