Responsibilities

• Assist patients with personal hygiene and daily living activities according to care plans.
• Provide CNA and home health/hospice aide services within training and certification.
• Monitor patients and report changes to the care team.
• Maintain accurate and timely patient care documentation.
• Follow infection control procedures and safety guidelines.
• Communicate professionally with patients, families and healthcare providers.
