Job summary

Berkshire Healthcare NHS Foundation Trust is seeking an experienced Assertive Outreach Occupational Therapist to deliver specialist OT assessment and intervention for individuals with complex mental health needs within a high-intensity, outreach setting.

The role involves proactive engagement, risk management, small caseloads, applying MOHO, using outcome measures, and working within a multidisciplinary team to place service users at the centre of goal setting.

Responsibilities

• Deliver specialist occupational therapy assessment and intervention for individuals with complex mental health needs.
• Provide assertive outreach interventions, including intensive and time-limited support during risk or disengagement.
• Use proactive engagement approaches to maintain contact with hard-to-engage individuals.
• Plan and deliver interventions with clear goals and outcome measures.
• Contribute to risk assessment and management with safety and positive risk-taking.
• Manage referrals and hold a small, high-intensity caseload within the MDT.
• Apply MOHO to inform clinical reasoning and interventions.
• Place service users at the centre of goal setting and involve carers where appropriate.
• Deliver culturally responsive and inclusive care and prescribed equipment per policy.
