Job summary

Responsibilities

• Coordinate with SFC and Occupational Therapist to provide support and intervention.
• Perform initial assessments, manage referrals, assess risk, and plan actions with participants.
• Co-produce meaningful goals with participants to enable participation and wellbeing.
• Deliver 1:1 interventions (e.g., anxiety management) and refer to external services as needed.
• Support and supervise volunteers/coaches; manage caseload and ongoing reviews.
• Ensure accurate records, data entry, and compliance with professional standards.
