Job summary

Personic Health is seeking a mobile Wound Care Nurse (RN or LPN) to provide in-home wound care across Montgomery, AL. This full-time, on-site role requires reliable transportation and daily travel to patients’ homes and facilities. Training is provided and paid for.

Responsibilities include wound assessments, Ultramist therapy facilitation, and coordination with the wound care team under clinical supervision. EMR experience and Alabama licensure are required.

Qualifications

• Proficient wound care knowledge and skills.
• Knowledge of safety and emergency procedures.
• Ability to document accurately in EMR.
• Strong communication with patients and care teams.

Responsibilities

• Perform assigned tasks under licensed clinical staff supervision.
• Utilize training on wound care principles and modalities.
• Coordinate care with medical providers and nurses.
• Carry out ABI assessments and wound swab collection.
• Travel daily to patients’ homes and facilities to provide in-home care.
