Job summary

Professional Wound Specialists seeks a mobile Wound Care Nurse Practitioner or Physician Assistant to travel to facilities and homes in the San Jose area, delivering on-site wound care management. You will assess wounds, implement tailored treatment plans, and collaborate with facility staff and PCPs. Must have CA NP license, 2+ years in wound care, and own vehicle.

Qualifications

• Current Nurse Practitioner certification.
• Active California NP license required.
• Minimum 2 years wound care experience.
• Own a vehicle and valid driver’s license; ability to travel.
• Strong communication and teamwork skills.
• Ability to work independently in mobile settings.

Responsibilities

• Conduct on-site wound assessments, including measurements and tissue evaluation.
• Develop and implement individualized wound care plans (debridement, dressings, adjunctive therapies).
• Collaborate with facility staff, PCPs, and care teams for comprehensive care.
• Educate patients, families, and caregivers on wound care techniques and prevention.
• Monitor progress and adjust treatment plans to promote healing.
• Maintain accurate documentation of assessments, interventions, and outcomes.
• Coordinate patient visits across multiple locations.
