Qualifications

• Valid LMSW license.
• Experience in health care setting, preferably home care or hospice.
• Excellent interpersonal and communication skills.
• Understanding of psycho-social issues related to families in crisis and end of life.
• Experience and competency to perform a psycho-social assessment.
• Experience and competency to work in the home or facility setting.
• Must be able to handle multiple tasks, be flexible and prioritize workload.
• Must be highly organized.

Responsibilities

• Provide social work services to patients and families and ensure adequate and timely coverage of patients.
