Rehab Without Walls Neuro Rehabilitation is seeking a Speech Language Pathologist to provide functional rehabilitation in home and community settings. The role focuses on treating patients where they live and engage in everyday activities, including kitchen tasks, grocery shopping, dining out, and workplace scenarios.

We offer a flexible schedule, hourly pay, travel reimbursement, and ongoing educational programs to support professional growth and advancement within the team.

Qualifikationen

• Master’s Degree in Speech Language Pathology from an accredited program.
• SLP license valid in practicing states.
• CPR certification per state regulations.

Aufgaben

• Perform evaluations and develop treatment plans for patients with speech, language, cognitive and swallowing disorders.
• Regularly re-assess effectiveness of treatment plans; attend staffing meetings and family conferences.
• Perform discharge evaluations and provide home/community safety recommendations and education resources.
• Communicate patient needs and progress to the treatment team, physician, patient and family.
• Coordinate with other disciplines to ensure collaboration and optimize patient outcomes.
