Conviva Senior Primary Care provides proactive, preventive care to seniors, including wellness visits and chronic condition management. The Care Coach coordinates care, conducts home visits, and connects patients with social resources to support health goals.

The role emphasizes patient-centered, culturally sensitive care, with a focus on transitions of care, adherence, and navigation across providers and settings.

Formación

• Healthcare professional with 3+ years of ambulatory or senior-care patient care.
• Ability to discuss chronic conditions and reinforce medication instructions.
• Comfort with home visits and community outreach.
• Experience in patient education and care coordination for high‑risk populations.

Responsabilidades

• Clinical screening and escalation with structured patient interviews.
• Perform home visits to assess living conditions and safety.
• Coordinate care between patients, providers, and community resources.
• Deliver chronic disease education using approved materials.
• Support transitions of care and follow-ups after hospitalizations.
• Engage community resources to support health goals.
• Deliver culturally sensitive, patient-centered care.
• Apply the 5Ms framework to identify barriers.
• Prepare and participate in High-Risk Rounds.
