Job summary

Remedy Home Health Care in North Orange County, CA is seeking a licensed Physical Therapist (PT) for our Home Health team. This Per Diem / PRN role offers flexible scheduling, autonomy, and the chance to deliver one-on-one rehabilitation in patients’ homes, senior living communities, and related settings. You’ll perform in-home evaluations, develop care plans, and document visits using EMR.

CA license and CPR are required; prior home health or geriatric experience is preferred.

Qualifications

• Licensed Physical Therapist (PT) in California.
• CPR Certification (AHA approved).
• Minimum 1 year of clinical experience (Home Health preferred).
• Experience with geriatric care, rehab or community-based care preferred.

Responsibilities

• In-home physical therapy evaluations and reassessments.
• Develop and implement personalized rehab plans.
• Improve strength, balance, mobility, and functional outcomes.
• Provide therapy in home health, assisted living, and community settings.
• Educate patients and families on fall prevention and safe mobility.
