Powerback Rehabilitation announces an opportunity for a Speech Language Pathologist in a home-based outpatient setting across Lowell, Tewksbury, and Chelmsford, MA. The clinician will evaluate, plan, and deliver speech-language services in patients\' homes or in care facilities, coordinating with families and care teams to maximize functional communication and swallowing outcomes.

Qualifications

• Must have valid Speech Language Pathologist license in the state(s) of practice.
• Must have a valid driver\'s license or reliable transportation to and from patients\' residences.

Responsibilities

• Supervise CFYs, temporary licensees, students and support personnel per state licensure requirements and professional standards.
• Adhere to department and patient treatment schedule and complete timely clinical and billing documentation.
• Attend and contribute to patient care, staffing conferences and related meetings.
• Make referrals to other disciplines to address patient needs.
• Collaborate with rehabilitation team and physician to select appropriate regimen.
• Adapt program/plan of care to patient needs and progress.
• Promote a safe environment and eliminate hazards.
• Coordinate care in home and community with the interdisciplinary team.
