Job summary

Your Health Organization is seeking a Nurse Practitioner to provide high-quality care in patients' homes across the Kingstree area. This full-time, salary-based role includes a monthly bonus and collaborative care with a multidisciplinary team.

Providers perform visits in their designated service area, supporting primary care, specialty services, and care coordination. A strong focus on patient education and outcomes is essential.

Qualifications

• Evaluate high-risk patients with complex needs and develop care plans.
• Coordinate care activities with interdisciplinary team to ensure continuity of care.
• Communicate with case managers, clinicians, pharmacists and health coaches to coordinate patient care.
• Document care management activities to support patient plans and regulatory compliance.
• Provide patients with education and resources related to conditions and self-management.

Responsibilities

• Evaluate high-risk patients with complex medical needs and develop individualized care plans.
• Coordinate care activities with other members of the interdisciplinary team to ensure continuity of care and optimal patient outcomes.
• Communicate regularly with case managers, clinical coordinators, social workers, specialists, pharmacists, health coaches, and other team members to coordinate patient care.
• Participate in interdisciplinary team meetings to discuss patient progress, treatment plans, and care coordination efforts.
• Foster effective communication and collaboration to facilitate seamless patient management and interdisciplinary teamwork.
• Meet staff efficiency expectations while providing high-quality care to patients in various settings, including homes, facilities, telehealth, clinics, and specialties.
• Document care management activities accurately and efficiently to support patient care plans and regulatory compliance.
• Provide patients with education and resources related to their conditions, treatment plans, self-management strategies, and preventive care measures.
