Job summary

Mission Mobile Medical seeks a Family Nurse Practitioner (FNP) or Physician Assistant (PA) to join our MHCS Department as a mobile primary care clinician. This full-time role conducts care at mobile clinic sites in Fremont, Gilroy, and Morgan Hill, CA with Monday–Friday daytime hours and no on-call duties.

You will provide comprehensive primary care across the lifespan, perform assessments, manage chronic disease, deliver preventive services, document in EHRs, coordinate care with partners, and

Qualifications

• Graduate of an accredited nurse practitioner program or accredited physician assistant program.
• Current Family Nurse Practitioner board certification or NCCPA certification.
• Current or obtainable California APRN/PA licensure.

Responsibilities

• Provide comprehensive primary care across the lifespan.
• Perform histories, physicals, and order/interpret diagnostic tests.
• Develop and monitor chronic disease care plans (diabetes, hypertension, asthma).
• Deliver preventive care and screenings.
• Maintain accurate EHR documentation.
• Coordinate care with colleagues and referral partners.
• Support mobile clinic operations and patient flow.
• Engage in community outreach and program development.

Education

Nurse Practitioner program (FNP) or Physician Assistant program

FNP board certification / California APRN licensure
