Job summary

Mission Mobile Medical is seeking a Family Nurse Practitioner (FNP) or Physician Assistant (PA) for mobile primary care in Fremont, CA. This full-time onsite role offers Monday–Friday daytime hours with no on-call obligations, delivering comprehensive care to patients across the lifespan via mobile clinics.

Base salary ranges $185,000–$195,000 plus benefits including 80 PTO hours after 60 days, 100% employer medical, 401K with up to 4% match, weekly pay, and quarterly profit sharing.

Qualifications

• FNP: graduate of an accredited nurse practitioner program; CA licensure obtainable.
• FNP: current family nurse practitioner board certification.
• For PA: graduate of an accredited physician assistant program; CA PA licensure obtainable.
• For PA: NCCPA certification.

General Tasks

• Primary Care Services: Provide comprehensive primary care, including evaluation and treatment of acute and chronic conditions, for patients across the lifespan.
• Patient Assessments: Obtain histories, conduct physical examinations, and order and interpret appropriate diagnostic tests.
• Chronic Disease Management: Develop, implement, and monitor care plans for conditions such as diabetes, hypertension, and asthma.
• Preventive Care: Perform screenings, provide patient education, and deliver preventive services within professional scope.
• Documentation: Maintain accurate and timely patient records in the electronic health record (EHR).
• Care Coordination: Collaborate with clinical colleagues and referral partners to ensure continuity of care.
• Mobile Clinic Operations: Support patient flow, clinic readiness, and appropriate administrative tasks in the mobile care setting.
• Community Outreach: Build relationships with local communities and support awareness of available healthcare services.
• Program Development: Contribute ideas and feedback to help strengthen and grow the mobile primary care program.

• Education & Licensure: For FNP applicants: graduate of an accredited nurse practitioner program, current family nurse practitioner board certification, and current or obtainable California NP licensure. For PA applicants: graduate of an accredited physician assistant program, current NCCPA certification, and current or obtainable California PA licensure.
• Experience: Primary care experience across the lifespan preferred; experience in community-based care is a plus.
• Clinical Judgment: Ability to manage a broad range of primary care needs with sound judgment, within applicable scope-of-practice and collaboration requirements.
• Technology: Comfort using electronic health records and telehealth or other clinical technology.
• Performance: Able and willing to provide world-class service to all stakeholders.

Salary & Benefits

Base salary: $185,000–$195,000. Monday–Friday daytime schedule; no on-call responsibilities.
