Mission Mobile Medical is seeking a Family Nurse Practitioner (FNP) or Physician Assistant (PA) for mobile primary care in Fremont, CA. The role is full-time with a base salary of $185,000-$195,000 and Monday–Friday daytime hours, no on-call.

Qualifications

• Education: NP program graduate with FNP certification and CA APRN license or PA program graduate with NCCPA cert and CA PA license.
• Experience: Primary care across lifespan preferred; rural/community-based care a plus.
• Clinical Judgment: Manage broad primary care needs with sound judgment and collaboration.
• Technology: Proficient with EHR and telehealth tools.

Responsibilities

• Deliver comprehensive primary care across all ages in mobile clinic setting.
• Obtain histories, perform exams, order and interpret tests.
• Develop and monitor chronic disease care plans (diabetes, hypertension, asthma).
• Provide preventive care, education, and screenings within scope.
• Document accurately in EHR and coordinate care with team.

Family Nurse Practitioner (FNP) / Physician Assistant (PA) – Mobile Primary Care

General Tasks

• Primary Care Services: Provide comprehensive primary care, including evaluation and treatment of acute and chronic conditions, for patients across the lifespan.
• Patient Assessments: Obtain histories, conduct physical examinations, and order and interpret appropriate diagnostic tests.
• Chronic Disease Management: Develop, implement, and monitor care plans for conditions such as diabetes, hypertension, and asthma.
• Preventive Care: Perform screenings, provide patient education, and deliver preventive services within professional scope.
• Documentation: Maintain accurate and timely patient records in the electronic health record (EHR).
• Care Coordination: Collaborate with clinical colleagues and referral partners to ensure continuity of care.
• Mobile Clinic Operations: Support patient flow, clinic readiness, and appropriate administrative tasks in the mobile care setting.
• Community Outreach: Build relationships with local communities and support awareness of available healthcare services.
• Program Development: Contribute ideas and feedback to help strengthen and grow the mobile primary care program.

Qualifications & Requirements

• Education & Licensure: For FNP applicants: graduate of an accredited nurse practitioner program, current family nurse practitioner board certification, and current or obtainable California APRN licensure. For PA applicants: graduate of an accredited physician assistant program, current NCCPA certification, and current or obtainable California PA licensure.
• Experience: Primary care experience across the lifespan preferred; experience in rural or community-based care is a plus.
• Clinical Judgment: Ability to manage a broad range of primary care needs with sound judgment, within applicable scope-of-practice and collaboration requirements.
• Technology: Comfort using electronic health records and telehealth or other clinical technology.
• Performance: Able and willing to provide world-class service to all stakeholders.

Salary & Benefits

• Base salary: $185,000.-$195,000. Monday-Friday daytime schedule; no on-call responsibilities.
• 80 PTO hours available after the first 60 days of service
• Medical insurance - 100% employer contribution, participating plans
• $10K Life insurance - 100% employer contribution
• Dental/Vision - employee contribution
• 401K - 120 days eligibility is fully vested with up to a 4% match
• Weekly payday, every Friday
• Quarterly profit sharing, up to 20% of annual wages, and an Open Book Management organization
