Job summary

Your Health seeks a skilled Primary Care Physical Therapist to serve Bluffton area patients, including homes, ALFs, ILFs, clinics, and Telehealth. This full-time role offers an 8-hour shift from 8:00 AM to 5:00 PM, Monday through Friday, with a guaranteed monthly bonus.

You will contribute to comprehensive care across settings and engage in community education. Strong patient care, communication, and team collaboration are required, with licensure in SC and CPT credentials.

• Ability to interact in a positive and motivating manner with patients and families.
• Will travel between offices, facilities, and other sites as needed.
• Experience working with health teams and community resources is valuable.

Responsibilities

• Obtain complete health and psychosocial history and document findings.
• Perform comprehensive physical assessments and provide health education and counseling.
• Arrange referrals to other health team members and community resources as needed.
• Use reporting mechanisms required by law while respecting patient confidentiality.
• Market and provide community education about services.
